Our new Support Planning and Brokerage Pilot launched this year.
This is 3 year part time pilot offering individualised Support planning and brokerage for adults with learning disabilities and their family carers, including young people moving through the transition process.
Our Support Planner and Broker works across a wide variety of agencies supporting individuals though the support planning and purchasing of their service so that they are enabled to meet the goals they set for themselves. Our Support Broker is responsible for supporting individuals to make decisions about their lives and what they may want to change.
Our project aims to achieve the following:
- Individual Service design – designing and developing individually tailored support packages to an agreed budget
- Recruitment of staff – assistance with the recruitment of staff
- Community development – helping people make connections in their own communities
- Monitoring services – monitoring services that have been purchased to ensure compliance with legal standards, etc
- Reviewing services – helping individuals review their services to their agreed timescales
- Signposting other information
- Benefit Advice – Support to ensure people are maximising their benefit entitlement
- Raise awareness and provide information about SDS
